You can register by either calling into the Surgery and completing a registration form here or by printing your own form.
To register you can click on the link below and complete the Patient Access online application form, or call into the Surgery for further information and your access details. Click here to Register Online/Book Appointments.
You can also Order Repeat Prescriptions, View elements of your coded medical record such as immunisations, results from samples sent for investigation, blood pressures, peak flow readings, problems and diagnosis.
You will need to provide photograph identification and proof of address to the Surgery in person, the link above will allow you to book one appointment and then a block will be placed on your records until the identification is received.
Tollerton Surgery provides a dispensing service for patients who live in Tollerton and the surrounding villages. Patients who live within 1 mile of the Pharmacy in Easingwold will be provided with a printed prescription slip to take to the Pharmacy to dispense their medication.

Repeat Prescriptions

The Practice has a fully computerised dispensing system that holds details of all medications prescribed to you by the Doctor. To order medication that is on a repeat prescription, we ask that this is done in writing and not over the telephone. You can either post your repeat prescription in the special postbox which is in the reception area of the Surgery, send it by fax or post to the Surgery. If you do not have the repeat slip from your previous medication, then please put it in writing giving your name, date of birth, medication required and dosage.

We are also able to offer an online prescription ordering service. To use this service you will need to call into the Surgery with photographic proof of identity and proof of address and request your individual PIN number and ID code. Once you are registered you will have access to all your repeat medication details and you can order online at your convenience by clicking on the link below.

The Practice requires a minimum of 48 hours to process and dispense your repeat prescription. This is to ensure that any queries regarding your prescription can be checked with the Doctor. Do not leave it until the very last minute to order your medication as we may need to order it from our supplier. If a request is put in over the weekend the 48 hours will start from Monday morning. Please allow a longer timeframe if you are posting your prescription.

At regular intervals your medication will need to be reviewed and you may be asked to make an appointment to see the Doctor to discuss this. Any queries regarding prescriptions are passed to the Doctor and this may mean that your request takes slightly longer to process. Our aim is to keep delays to a minimum, to try and ensure that your prescription or drugs are available within 48 hours.

The practice operates a 28 day prescribing policy (apart from HRT and contraception). This means that the maximum amount of medication you will be given at any one time will last 28 days/one month. This is in line with the Government policy to reduce wastage across the NHS as a whole.
Making Appointments

Appointments may be made during Surgery opening hours in person or by telephoning the Surgery on 01347 838231. Doctors appointments are available to pre book up to 3 weeks in advance.

If you need to make an urgent / emergency appointment, please call at 8.00 am for a same day appointment. Please note that these appointments operate on a first come, first served basis. The Receptionist will need to take some details from you to pass to the Duty Doctor who will call you back to discuss your emergency. Following this, the doctor will be able to offer you advice, prescribe suitable medication or offer you an emergency appointment. Should the Doctor think that it is inappropriate use of the emergency appointment system, you may then be asked to make a routine appointment.

GP Home Visits

Patients who are too ill to attend the surgery can request a home visit. Please telephone the surgery as early as possible, before 10.30 am and leave your telephone number in case the Doctor wishes to speak to you. You will be asked to give some indication of the nature and urgency of the illness.
Lateness

It is the Practice policy that if you are more than 10 minutes late you may have to re-book an appointment. If your situation is urgent then you may have to wait to see the doctor as an emergency.

Weekend and Out of Hours

After 6 pm and over the weekend there is an out of hours service for providing medical care for our patients. If you need a Doctor in an emergency outside the normal Surgery opening times then you should contact 111.
